Our church is just moving towards formal membership. I searched for a long time for some good examples of membership transfer forms so I didn’t have to start from creating them from scratch. I didn’t find any so I thought I would post them here. Hope they help!
Statement of Membership: We use this form for transfers to churches that are non-denom or part of other denominations.
Transfer of Membership: We use this form for transfers to churches with in our own denomination.
